Lab testing consent

Read this before you order. When you place an order you agree to it, along with our Terms of Service.

1. Consent to testing

You are asking for the lab tests you selected to be performed on a blood or other specimen taken from you. You agree to have that specimen collected at a Quest Diagnostics or Labcorp location, and you agree that the laboratory may run the tests you ordered and report the results to Labs and Consulting and to you.

You are ordering these tests for yourself. You are 18 or older. The name and date of birth you give must be your own and must match your identification, because the laboratory uses them to match your specimen to your result.

3. Talk to a doctor about your results

Take your results to your own physician or another licensed clinician and go through them together. Do not start, stop, or change any medication, supplement, diet, or treatment based on a number on this report. If a result concerns you, or you feel unwell, contact your doctor. If you think you are having a medical emergency, call 911 or go to the nearest emergency room. Do not wait for a lab result.

4. No physician-patient relationship

Your order is authorized by a licensed physician through an independent physician network, so that the laboratory can lawfully run it. That physician does not evaluate you, does not examine you, does not review your results with you, and does not provide you with care or advice. You are not their patient. No physician-patient relationship is created with that physician or with Labs and Consulting.

5. The limits of self-ordered testing

Lab tests are useful, and they are also limited. You should know the following before you order.

• A test can be wrong in either direction. A false positive suggests a problem that is not there. A false negative misses one that is. Neither is rare enough to ignore.
• A result is a snapshot of one moment. Values move day to day and even hour to hour.
• Fasting, hydration, recent exercise, illness, time of day, supplements, and medications all move results, sometimes a great deal.
• Reference ranges are set by the laboratory and are not a statement about your individual health. A value outside the range is not automatically a problem, and a value inside it is not automatically fine.
• Testing without a clinician's guidance can cause unnecessary worry, or false reassurance that delays care you actually need.

6. Risks of the blood draw, and your assumption of them

Having blood drawn carries some risk. The common ones are bruising, a hematoma (blood collecting under the skin), soreness at the site, lightheadedness, and fainting. Less often, an infection or excessive bleeding can occur. Rarely, more than one attempt is needed, or a specimen is insufficient and you must return to be drawn again.

The draw is performed by the laboratory's staff at the laboratory's location, not by Labs and Consulting. You accept these risks, and you agree that any question about how a draw was performed is between you and the laboratory. 7. Getting your results if something goes wrong

Your results are held by the laboratory that performed your tests, as well as by us. If a result does not reach you, you can obtain it directly from the laboratory. Register an account with Quest Diagnostics or Labcorp under the same name and date of birth you gave on your order, and your results will appear there.

8. State availability

Some states regulate self-ordered lab testing in ways that limit who may use a service like ours. We do not serve New Jersey, New York, or Rhode Island. Which specific tests are available to you may also depend on your state and on which laboratory serves your area.

9. Your acknowledgment

By placing an order you confirm that you have read and understood this consent, that you are 18 or older, that you are ordering for yourself, that you have had the chance to ask us questions, and that you agree to the terms above and to our Terms of Service and Privacy Policy. If you do not agree, do not place an order.
